IRA Distribution Request Forms

Please be advised that we will be unable to process any distribution requests without first having received a completed IRA Adoption Agreement, accompanied by a clear copy of a valid government issued photo identification.

If you have not returned an IRA Adoption Agreement, you may obtain one by clicking here.  Failure to provide a completed IRA Adoption Agreement and legible photo identification will dramatically delay your distribution request.

If you have previously completed an IRA Adoption Agreement and wish to proceed with your distribution request, you may access the appropriate form(s) below.

You will need to complete a Distribution Request form for each type of account that makes up your arrangement with AutoRollovers.

If your arrangement with AutoRollovers is strictly a Traditional IRA (most individuals), then you will only need to complete the Traditional IRA Distribution Request Form.

If you have both Traditional and Roth IRA Accounts with us, you will also need to complete a Roth IRA Distribution Request Form.
