• Plan Sponsor Agreement
  This agreement authorizes AutoRollovers to establish IRAs on behalf of your Plan Participants. This must be signed by a named fiduciary, prior to remitting any rollovers to us. The linked document is a SAMPLE, only.  For an useable form, please e-mail autorollovers@bpas.com.

• IRA Rollover Form
  This form must be completed and signed for each IRA rollover being remitted. The completed form should accompany or precede any remittance of rollover funds.

• IRA Account Funding Instructions
  Detailed funding instructions for sending IRA rollover proceeds to AutoRollovers.

• IRA Accountholder Agreement
  The IRA Disclosure and Custodial Agreement, as referenced in the Plan Sponsor Agreement. This document describes the terms of the IRA, which are enforceable by the accountholder.
